Responsibilities:
- Coordinating patient care within an acute care setting.
- Collaborating with an interdisciplinary team.
- Floating to different units within the main hospital campus.
- Potentially floating to a second campus located 1.7 miles away.
- Working with various patient populations across different units (e.g., ED, ICU, ITA's, Peds, Specialty Units).
- Performing patient assessments, medication administration, and treatment planning (as applicable to the Case Management role).
Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of employment.
- Minimum one year of experience in a hospital setting.
- Clinical assessment, medication administration, and treatment planning skills (as applicable to the Case Management role).
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
Preferred Skills & Certifications:
