Responsibilities:POSITION SUMMARY: As the Case Manager, you are a member of a multidisciplinary team, utilizing your skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to Adult Community Clinical Services (ACCS) clients under the clinical supervision of the team leader, assistant team leader, and other mental health professionals. A team member equally shares the responsibility to perform various activities and interventions that support (ACCS) clients in the community.
